00:00:00I've heard you talk about the need for young men to retrain their penis.
00:00:03Yes.
00:00:03What's going on there?
00:00:05So here's the basic problem.
00:00:08I'm a teenage boy, and I discover pornography, and I have an erection, and at some point,
00:00:14I start masturbating.
00:00:15And then this is becoming a problem now because my access to pornography is at an all-time
00:00:22high.
00:00:23My access to a healthy sexual relationship is at an all-time low.
00:00:28So whereas, you know, so now what's happening is for maybe 15 years, I am using a certain
00:00:39kind of stimulation for my penis.
00:00:42And just to give you, okay, so I'm using a certain kind of stimulation for my penis.
00:00:47And then the body gets used to that, right?
00:00:50So in the same way that, like, if I eat spicy food starting when I'm 13, like, I can handle
00:00:54spicy food when I'm 15.
00:00:56So the body gets used to caffeine.
00:00:59It gets, the whole point of the body is that it adapts to whatever you give it.
00:01:02So we are literally, we're training our body to tolerate spicy food.
00:01:06We're training our body to run a marathon.
00:01:09We're training our genitals and our penis to ejaculate in certain circumstances.
00:01:15So we see a shockingly high level of erectile dysfunction.
00:01:21I'd learn from you, 40% of men are dealing with erectile dysfunction.
00:01:25Yeah.
00:01:26So I think it's 30% of people under the age of 30, which erectile dysfunction, maybe I
00:01:30said 40%.
00:01:30I don't remember the exact statistic, but erectile dysfunction used to be an old man's disease.
00:01:34It's not anymore.
00:01:36And what a lot of people don't realize, do you know what erectile dysfunction is?
00:01:39No.
00:01:39Okay.
00:01:40So this is-
00:01:41Obviously.
00:01:44Okay.
00:01:48Too easy.
00:01:50Yeah, that's a good one.
00:01:51So erectile dysfunction is defined as the inability to maintain an erection through the completion
00:01:58of the sexual act.
00:01:59Okay.
00:02:00Okay.
00:02:01So the interesting thing about the young men, 30% of people under the age of 30 who have
00:02:05erectile dysfunction, it's not that they can't get hard.
00:02:08It's that they can't stay hard enough to finish if they're having intercourse, let's say.
00:02:12Okay.
00:02:13And there's a really simple reason for that.
00:02:16The-
00:02:17I was going to ask a couple of questions, but I doubt you have the answer.
00:02:20So the PSI, the pounds per square inch of your hand, okay, the amount of force that you can
00:02:29exert with your fist is about 130.
00:02:33Now, there's some problems with that because you're actually, you measure hand grip strength
00:02:39not by force, but by Newtons, but sorry, not by PSI, but by Newtons, but there's a rough
00:02:43conversion, okay?
00:02:44It's about 130.
00:02:46The PSI of the vagina is like 30.
00:02:49So if you just, if you just, in this, you may not know, but you know, the vagina exerts
00:02:57a certain amount of pressure and tightness.
00:03:00And then the hand exerts a different level of tightness and they're not even in the same
00:03:04vein.
00:03:05So basically what happens with a lot of dudes who spend 15 years of their life watching
00:03:09pornography and masturbating, it's not just about gender stuff or the jiggliness or the
00:03:14angles of porn.
00:03:15There's all that stuff too.
00:03:16There's a very simple physiologic thing where this is what you need to finish.
00:03:20You need a very high level of stuff.
00:03:23And I mean, I've worked with men in their 60s who will not be able to climax through oral
00:03:29sex or vaginal intercourse.
00:03:31And so when they have, when they engage in sexual intercourse, either they have to finish
00:03:36using their hand or they have to engage in anal sex.
00:03:39Those are the two things that work.
00:03:42And so this, it's just physiology.
00:03:44It's just what you've trained your penis to respond to.
00:03:47What's the pounds per square inch of an asshole?
00:03:51I don't know, but I...
00:03:53This is important research.
00:03:54I imagine it's probably north of a hundred.
00:03:57Really?
00:03:58Yeah.
00:03:58Wow.
00:03:59Yeah.
00:03:59Especially if you...
00:04:01Yeah.
00:04:02You know?
00:04:03Yeah.
00:04:05So, I mean, it's got to be high.
00:04:07I mean, it's certainly way higher than...
00:04:09I mean, there's a...
00:04:11Anyway, a couple sphincters in there, but they're quite strong.
00:04:14Right.
00:04:15But so this is what's going on.
00:04:17So men have basically...
00:04:18And there's all kinds of other issues, like cardiovascular issues, sedentary lifestyle.
00:04:23That's a good question here, because I think when people hear erectile dysfunction in
00:04:27men, what they probably go to is like an uninformed person or even a well-informed person.
00:04:35They go, well, that is low testosterone.
00:04:40That is obesity.
00:04:44That is mental stimulus desensitization due to extreme fishery and runaway acts in porn and
00:04:56seeing unrealistic or tail-end representations of women in porn.
00:05:02That is due to psychological nervousness.
00:05:07Far fewer will go to how much nitric oxide are you consuming from your fucking leafy greens
00:05:14and your beetroots.
00:05:15What's the level of cardiac health that you've got?
00:05:19What's the death grip comparison between your right hand and a vagina?
00:05:25Of the culprits causing erectile dysfunction among men, young men, how would you sort of partition
00:05:33off that pie?
00:05:34So this is something that I've been dragged kicking and screaming into becoming an expert
00:05:41in.
00:05:42So let's start with testosterone.
00:05:44Specialist subject.
00:05:44Let's fucking go.
00:05:45This is the biggest scam that is being perpetrated against men.
00:05:48Okay.
00:05:49So around the year 2000, the average testosterone for a man was about 612 to 690 nanograms per
00:05:59deciliter.
00:05:59Okay.
00:06:01So 20 years later, the average testosterone for a man has dropped to about 400.
00:06:07Okay.
00:06:07That's significant in a short space of time.
00:06:09It is significant in a short space of time and it is well within the acceptable limits.
00:06:15Right.
00:06:15So it, so, so has testosterone dropped?
00:06:19Yes.
00:06:19Has it dropped by a third?
00:06:21Yes.
00:06:22Does this cause physiologic problems?
00:06:24Almost certainly not.
00:06:27Second thing.
00:06:28So what's the testosterone drop due to?
00:06:30There is an effect from obesity, but it is, even if you factor out obesity, there's still
00:06:35a testosterone drop.
00:06:38Next thing.
00:06:39I have a theory on that.
00:06:40Okay.
00:06:40Do you want to hear my theory?
00:06:40Sure.
00:06:41Do you want to keep going?
00:06:41Sure.
00:06:41Okay.
00:06:42So this is my theory.
00:06:44Men's testosterone is mediated by their environment.
00:06:46If we had a bunch of weapons around here now, our testosterone would be higher.
00:06:50And if we started picking them up and waving them around, our testosterone would be higher.
00:06:52Not much, but like a little bit.
00:06:53Another thing that mediates male testosterone is the fertility of the women that they're near.
00:06:59But if you're spending a lot of time around women who are giving off the cues, largely pheromones,
00:07:04but also visual cues of being fertile, that will increase testosterone.
00:07:09Not by an insignificant amount.
00:07:10But if you were a man that somehow was stranded on a desert island, some hunter-gatherer man,
00:07:16stranded on a desert island with a bunch of children, a bunch of grandmothers, your
00:07:19testosterone would be less.
00:07:20Okay.
00:07:21I think, and I've spoken to Dr. Shana Swan about this, et cetera, et cetera.
00:07:27One of the X factors, my like tinfoil hat X factor thing, is the artificial suppression
00:07:32of fertility by women increasing their use of hormonal birth control is causing, and the
00:07:38solitude of men spending less time around women is a contributor that hasn't been fully factored
00:07:45in to the decrease in testosterone.
00:07:47That's a fascinating, especially the birth control.
00:07:49Don't fucking test me, dude, on this tinfoil hat stuff.
00:07:53That's what I'm about.
00:07:53Yes.
00:07:54That's what I'm about.
00:07:54Dr. fucking K.
00:07:55Giving me-
00:07:56So I know we have a lot of important things that matter to people to talk about, but one
00:08:00day we have to put our tinfoil hats on and just go off the rails.
00:08:03I have a hat that says-
00:08:06Make autism great again?
00:08:07Yes.
00:08:07Okay.
00:08:07Interesting.
00:08:08Cool.
00:08:09So first one was testosterone scam.
00:08:11Doesn't really seem to, is there a level, but question, is there a level of testosterone
00:08:14that it gets below where erectile function begins to be impacted?
00:08:18Yes.
00:08:18That's where I'm going next.
00:08:19So 5% of erectile dysfunction is due to testosterone deficiency.
00:08:25Specifically, we call that hypogonadism.
00:08:27So basically 95% of erectile dysfunction is not due to testosterone.
00:08:31Don't worry about your testosterone.
00:08:32Okay.
00:08:34And I think that the things that you said are certainly factors for lowering testosterone.
00:08:39I think there's also really other simple little things like sedentary lifestyle.
00:08:43We've become more sedentary.
00:08:45I think the largest, don't skip leg day, right?
00:08:47So one of the greatest endogenous signals that stimulate testosterone production is like
00:08:54stressing your quadriceps muscles.
00:08:56Big muscles.
00:08:56Yeah.
00:08:57So the bigger your muscles and quads are about as big as they get.
00:09:00Yep.
00:09:00So don't skip leg day.
00:09:02Huge.
00:09:03The other thing that you touched on, which I think a lot of people forget, the penis is
00:09:07not like a endocrine organ.
00:09:11It's a vascular organ, right?
00:09:13So testosterone is a hormone.
00:09:15It travels in very small quantities all throughout your body and does all kinds of things everywhere
00:09:20in your body.
00:09:21The penis is first and foremost a vascular organ.
00:09:24It's basically a tube that fills up with blood.
00:09:27So the number one thing that improves, actually, I don't know if this is true, but if-
00:09:33One of the things.
00:09:34Yeah.
00:09:35So one of the things that is very, very, very important for penile retraining and penis
00:09:38health is cardiovascular health.
00:09:40The stronger your heart is, the stronger your cardiovascular system is, the healthier your
00:09:44penis will be, the longer you will be able to maintain an erection.
00:09:47Even the size of your erection depends on how much blood goes into the corpus cavernosum
00:09:53and the corpus spongiosum, which are anatomical parts.
00:09:56There's also valves that become really important.
00:09:58And this is where you mentioned nitrous oxide.
00:10:00So if we look at sildenafil, which is Viagra, it's a nitrous oxide.
00:10:06It increases the levels of nitrous oxide, basically.
00:10:08It's a phosphodiesterase 5 inhibitor.
00:10:11So it prevents the breakdown of nitrous oxide.
00:10:13And nitrous oxide is what causes the penis to swell.
00:10:17So cardiovascular health is huge.
00:10:20And then there are, so if we're talking about this penile retraining regimen, there are a
00:10:25couple of things that you have to do.
00:10:27The first is it takes about 10 days to two weeks for things in your body to start shifting.
00:10:35Okay.
00:10:36So first thing is, if we look at about 50% of men will have a nocturnal emission after
00:10:43seven days without any ejaculation.
00:10:4650%?
00:10:46Yeah.
00:10:47No way.
00:10:48I think it's ballpark 50%.
00:10:49I'm not 100% sure.
00:10:50I love the term nocturnal emission as well.
00:10:52It's my fucking favorite.
00:10:53So what that means is if you don't have intercourse and you don't tug one out, the body starts to
00:10:57recalibrate and will say 50% of men is like, okay, like we got to get rid of some of this
00:11:02stuff.
00:11:02So that'll happen.
00:11:03So once you hit two weeks, then, you know, the likelihood of nocturnal emission goes up.
00:11:08And this two-week number is like magical in the body.
00:11:12So even if you look at the hypertrophy of muscles, so if you stop working out, if you don't
00:11:19lift weights, after about 10 days is when you really start to see a loss of the mass of your
00:11:26muscles.
00:11:27So this two-week period is really important.
00:11:29So I think if you're like retraining your penis, you got to do it.
00:11:31You got to give yourself two weeks.
00:11:33Okay.
00:11:35So don't skip penis day.
00:11:39So penis day is leg day.
00:11:41So you want to work out your legs.
00:11:42You want to improve your cardiovascular health.
00:11:44The more that you work out, like the healthier your penis will become.
00:11:47Second thing is you need to abstain.
00:11:50I think it's probably a good idea to abstain from if you really want to retrain your penis.
00:11:55And usually the situation in which this becomes acutely important is if you start a sexual
00:12:01relationship.
00:12:02We'll get to the anxiety and stuff at the end because that's a whole conversation.
00:12:06But so when you're, you know, if you're going to get laid for like 10 days, like don't tug
00:12:13one out.
00:12:13We'll get to some other conspiracy tinfoil hat stuff about, you know, premature ejaculation
00:12:18and stuff like that.
00:12:18But so give yourself about two weeks and then, and then, you know, as dudes know, like if
00:12:24you haven't tugged one out, you're going to not fast, which is like the goal.
00:12:27Okay.
00:12:27So that's what we don't want to avoid nutting fast.
00:12:30Get to that in a second.
00:12:33But it's okay to not fast.
00:12:34That's actually like a good thing to do.
00:12:36And then if your penis is, if you're able to complete a sexual act, um, then, you know,
00:12:43you're on the right path because the problem with erectile dysfunction is you can't maintain
00:12:46an erection through the completion of the sexual act.
00:12:50So next thing that we want to do is loose grip and lots of lube.
00:12:56So we want to mimic the 30 PSI.
00:12:59Okay.
00:12:59So this is something that is very, very difficult and frustrating for dudes to do, which is why
00:13:05I recommend the buddy system, right?
00:13:07So if you have trouble doing, doing a loose grip, you can ask a friend of yours to use some
00:13:13lube and, and make sure that the grip stays loosened.
00:13:18Such a left turn, such a left turn.
00:13:21Hey, dude, sit in the, sit in the cut chair in the corner and warn me if I get above 30 PSI.
00:13:25Yeah.
00:13:26So, so, so it's, it's hard, right?
00:13:28So it's, I mean, it's not hard.
00:13:29Or not, which is the issue.
00:13:30Yeah.
00:13:31All right.
00:13:31So penis jokes galore.
00:13:32Let's go.
00:13:32All right.
00:13:33So, um.
00:13:35But yeah, so, so you have to loosen your group grip and use lots of lube.
00:13:39Yep.
00:13:40Um, that'll, that'll do pretty well.
00:13:41Then there's the pornography stuff where I think the, the more you, if you're waiting a
00:13:47week, right.
00:13:48And then you are engaging in masturbation best.
00:13:51If you don't use porn and let your thoughts do the work for you.
00:13:55And that opens up a whole other really interesting thing, which is like addressing premature
00:13:59ejaculation or inability to ejaculate is your mind is like an incredibly important organ to
00:14:07control your sex.
00:14:10Point and shoot.
00:14:11I don't understand.
00:14:13Oh, point and shoots.
00:14:13The, uh, parasympathetic sympathetic.
00:14:16Yeah.
00:14:16Seesaw.
00:14:17That's the way that they get taught at med school in the UK.
00:14:19Yeah.
00:14:19Yeah.
00:14:19So point and shoot, we get taught the same thing.
00:14:21Oh.
00:14:22So like, so when I have patients who struggle with premature ejaculation, one of the things
00:14:27that I'll tell them to do is like, think non-sexual thoughts.
00:14:29So math problems is really well, like do like factors, like prime factor analysis, things
00:14:33like that.
00:14:34Okay.
00:14:34And cause if you, if you think about like, if you're a dude who's had, you know, good sex
00:14:39before, you know, like getting into it is like a big part of it.
00:14:43Yeah.
00:14:43So, so even when I talk to, um, you know, partners or people who are engaged or want to
00:14:50do some certain kinds of sexual exploration with things like anal intercourse and stuff like
00:14:54that, the mentality becomes really important for sex.
00:14:58Like, so what, what is going on in your head, right?
00:15:01So you said point and shoot, which means parasympathetic and then sympathetic.
00:15:05Parasympathetic is relaxation.
00:15:06So this is why for, if you, if you're trying to engage in, in sex, you're trying to engage
00:15:12in the sex, right?
00:15:13So especially, I mean, this is really true for women, but it's also true for men, especially
00:15:17if they have anxiety induced issues, you want to be really, really, really relaxed.
00:15:21Right.
00:15:22Um, and so if you're relaxed, blood flow increases to your genital organs, and then you start
00:15:26to have thoughts and those thoughts will excite you.
00:15:29And that excitement in your mind, the dirty thoughts that you have will help you climax.
00:15:36A quick aside, you've probably heard me talk about Element before, and that's because I've
00:15:39started every single morning the same way for the last five years now, which is a cold glass
00:15:43of water with Element in it.

Key Takeaway

Erectile dysfunction in young men stems primarily from high-pressure masturbation habits and vascular health rather than low testosterone, requiring a two-week retraining process involving reduced physical grip pressure, improved cardiovascular exercise, and mental relaxation.

Highlights

  • Approximately 30% of men under the age of 30 experience erectile dysfunction, defined as the inability to maintain an erection through the completion of sexual intercourse.

  • The human fist exerts around 130 pounds per square inch (PSI) of force during masturbation, whereas the vagina exerts approximately 30 PSI.

  • Testosterone levels in men dropped by roughly one-third between 2000 and 2020, but testosterone deficiency accounts for only 5% of erectile dysfunction cases.

  • Penile erection depends primarily on vascular health and nitric oxide, making cardiovascular exercise and leg training critical for maintaining function.

  • Retraining penile sensitivity requires at least two weeks of abstaining from high-pressure masturbation, adopting a loose grip, using ample lubricant, and avoiding pornography.

Timeline

Desensitization from Excessive Grip Pressure

  • Erectile dysfunction among young men typically manifests as losing an erection before finishing rather than an inability to achieve one.
  • Prolonged masturbation using a high-pressure grip conditions the penis to require extreme physical stimulation to ejaculate.
  • A human hand exerts up to 130 PSI of force, while a vagina exerts approximately 30 PSI.

High access to pornography combined with lower rates of sexual activity leads young men to condition their physical response over years of masturbation. The physical force exerted by a hand grip far exceeds the natural pressure of sexual intercourse. Over time, the body adapts to this artificial standard, rendering normal vaginal or oral contact insufficient for reaching climax.

Debunking the Testosterone Myth and Prioritizing Vascular Health

  • Average male testosterone dropped from 612-690 ng/dL in 2000 to around 400 ng/dL 20 years later, yet hypogonadism causes only 5% of erectile dysfunction.
  • The penis functions primarily as a vascular organ reliant on blood flow and nitric oxide rather than an endocrine organ.
  • Leg exercises like squats trigger significant endogenous testosterone production by stressing large muscle groups.

Marketing often highlights low testosterone as the primary cause of sexual dysfunction, but 95% of cases are unrelated to hormone deficiency. Because erections rely on blood filling the erectile tissue, cardiovascular health and proper circulation are the true physiological drivers of penile function. Medications like sildenafil work by inhibiting phosphodiesterase-5, which prevents the breakdown of nitric oxide and expands blood vessels.

Protocol for Penile Retraining and Psychological Regulation

  • Resetting the body's neuromuscular adaptation requires a minimum period of 10 to 14 days.
  • Retraining involves using a loose hand grip, high amounts of lubricant, and relying on mental imagination instead of pornography.
  • Erection maintenance relies on parasympathetic relaxation, whereas climax is driven by sympathetic arousal.

A two-week period of abstinence allows the reproductive system to recalibrate, often triggering nocturnal emissions within the first week. When masturbating after this period, mimicking natural conditions with light pressure and heavy lubrication helps lower the required sensory threshold. Additionally, managing anxiety is critical because blood flow increases under parasympathetic relaxation, while mental focus controls the timing of climax.
